The Clinical Significance of Serum Type IV Collagen Concentrations in Patients in Patients with Diabetes Mellitus




Abstract
Background : In long-term diabetes mellitus, thickening of basement membrane in capillaries and small vessels is a characteristic lesion and plays an important role in the progression of diabetic microangiopathy.
Methods : In order to evaluate the clinical applicability of serum level of type IV collagen as a biochemical marker for the early diagnosis of diabetic microangiopathy and monitoring the progression of diabetic complications, serum type IV
collagen
levels were measured by sandwich enzyme immunoassay method in 83 diabetic subjects and 51 control subjects.
Results : Serum type IV collagen levels in diabetic patients (203¡¾103 ng/mL, mean¡¾SD) were significantly increased in comparison to those in the control group (117.5¡¾47.2ng/mL). In the patients with diabetic microangiopathy, serum type IV
collagen
levels became higher as clinical signs of nephropathy or retinopathy worsened. Serum type IV collagen levels in diabetic patients significantly correlated with blood urea nitrogen and serum creatinine (correlation coefficients : 0.62 and 0.70,
respectively). Serum type IV collagen levels in diabetic patients showed a tendency to increase significantly in proportion to extent of proteinuria, however, no significant correlation between serum type IV collagen levels and urinary
albumin/creatinine ratio in these groups was observed. As far as concerning the predictor of diabetic nephropathy, serum type IV collagen level could not be substituted for microalbuminuria which has already been used for this purpose.
Conclusions : In summary, serum type IV collagen level could be a useful noninvasive biochemical marker for monitoring diabetic microangiopathy which mainly involves abnormal metabolism of basement membrane. (Korean J Clin pathol 1994;
